External Client Care Coordinator | Full-Time
Service Areas: Pottsville, Schuylkill Haven, Saint Clair, Minersville, Ashland, Girardville, Frackville, New Philadelphia, New Ringgold, Tamaqua, and surrounding communities.
Build relationships that make a difference. Comfort Keepers is looking for an organized, people-focused External Client Care Coordinator to help connect families with quality in-home care while supporting an outstanding client experience.
What You'll Do
- Build strong relationships with clients, families, caregivers, and referral partners
- Conduct in-home consultations to understand client needs and recommend services
- Coordinate care plans and caregiver schedules to ensure quality service
- Follow up with clients to maintain satisfaction and address concerns
- Collaborate with office staff to provide seamless care coordination
- Maintain accurate client records and required documentation
- Develop partnerships with healthcare providers and community organizations
- Support local networking events and community outreach initiatives
- Help identify opportunities to improve the client experience
What We're Looking For
- Excellent communication and relationship-building skills
- Strong organizational and time-management abilities
- Ability to prioritize multiple tasks in a fast-paced environment
- Professional, compassionate, and customer-focused mindset
- Comfortable using computer systems and office software
- Experience in healthcare, client services, scheduling, or coordination is a plus
