  • Develop and update Crisis/Safety Plans and Natural Disaster Plans, ensuring 24/7 availability for family support during crises.

  • Maintain ongoing knowledge of community resources, develop relationships with service providers, and identify supports that strengthen the family’s capacity and independence.

  • Evaluate Plan of Care effectiveness regularly and collaborate with the Child and Family Team to adjust interventions as needed.

  • Maintain a caseload not exceeding 15 partnered families.

Wraparound Philosophy:

The wraparound approach emphasizes:

  • Building on strengths to meet needs

  • One family, one plan

  • Best fit with culture and preferences

  • Increased parent choice and family independence

  • Care for youth in the context of their families

  • Never giving up
