Case Manager/Care Coordinator Level 2 – OBMAT

Position Summary

Southern Highlands Community Mental Health Center is seeking a compassionate and organized Case Manager/Care Coordinator Level 2 to support individuals participating in our Office-Based Medication-Assisted Treatment (OBMAT) program.

The OBMAT Case Manager provides comprehensive care coordination and support to individuals receiving treatment for substance use disorders. This position assists clients in accessing the services and resources necessary to support treatment engagement, recovery, stability, and overall well-being.

The Case Manager works collaboratively with clients, treatment providers, community organizations, and other members of the multidisciplinary treatment team to coordinate services, monitor progress, address barriers to care, and promote continuity of treatment.

Education and Experience Requirements

Applicants must meet one of the following qualification pathways:

  • Bachelor’s or master’s degree in a human-services-related field that meets the requirements established by the West Virginia Board of Social Work;
  • Licensed psychologist with a master’s or doctoral degree from an accredited program; or
  • Licensed registered nurse.

Preferred Skills and Abilities

  • Strong communication, organization, documentation, and time-management skills.
  • Ability to work effectively as part of a multidisciplinary treatment team.
  • Ability to establish professional, supportive relationships with individuals from diverse backgrounds.
  • Knowledge of community resources, behavioral health services, substance use disorder treatment, and recovery support services.
  • Ability to maintain professional boundaries and protect confidential client information.
  • Compassionate, respectful, and person-centered approach to care.
  • Ability to manage multiple priorities and respond appropriately to changing client needs.
