And you understand that accurate OASIS documentation and a strong initial plan of care can influence everything that happens throughout the patient's home health episode.
That's the expertise we're looking for.
What You'll Do
- Complete weekend Start of Care admissions for new home health patients.
- Perform comprehensive skilled nursing assessments.
- Complete OASIS and admission documentation accurately and on time.
- Review medications, diagnoses, symptoms, safety concerns, and caregiver needs.
- Develop and initiate individualized plans of care.
- Provide patient and caregiver education.
- Identify additional clinical and therapy needs.
- Coordinate care with physicians, nurses, therapists, and other interdisciplinary team members.
- Communicate significant clinical findings and changes appropriately.
Who We're Looking For
This is not intended to be an entry-level home health opportunity. We're looking for an RN who can confidently manage the complexity of an admission in the field.
Required Qualifications
- Active Minnesota Registered Nurse license.
- Current CPR certification.
- Previous home health nursing experience.
- Strong OASIS knowledge.
- Start of Care or admission experience strongly preferred.
- Strong assessment and clinical decision-making abilities.
- Excellent documentation and communication skills.
- Valid driver's license, active insurance, and reliable transportation.
Already Working Full-Time? That's Okay.
This opportunity can make sense for an RN who enjoys their current weekday position but wouldn't mind adding premium weekend income.
It can work for a part-time nurse looking to add additional visits.
Or it can be an excellent fit for an experienced home health clinician who simply enjoys admissions, assessments, and clinical problem-solving more than maintaining a traditional caseload.
