Job Summary:
The Population Health Registered Nurse (RN) functions within the nursing scope of practice under the direction and supervision of the Provider (physician, PA or ARNP) and is responsible for assisting in providing care and implementing delegated activities of care.聽An understanding of contemporary nursing principles and skills must be demonstrated, unique to their clinical assignment.

路聽聽聽聽聽聽聽聽Supports population health initiatives focusing on wellness, prevention and chronic care management.
路聽聽聽聽聽聽聽聽Collaborates, communicates and coordinates with the Provider and other health care team members in planning and
implementing patient care.
路聽聽聽聽聽聽聽聽Understands and has knowledge of Medicare documentation, billing and reporting requirements associated with Annual
Wellness Visits (AWV), Chronic Care Management (CCM) and Transitional Care Management (TCM) services to include
patient calls and calculation of billable time.
路聽聽聽聽聽聽聽聽Performs face to face visits such as Medicare Annual Wellness Visits.
路聽聽聽聽聽聽聽聽Conducts injection clinics as appropriate (e.g. allergy injections, INR).
路聽聽聽聽聽聽聽聽Performs testing as directed by a provider (e.g. EKG)
路聽聽聽聽聽聽聽聽Helps providers close patient care gaps (e.g. colonoscopies, mammography, A1c, etc.)
Education and/or Experience Requirements:
路聽聽聽聽聽聽聽聽Graduate of an accredited school of Nursing.
路聽聽聽聽聽聽聽聽Valid license as RN in the State of Washington.
路聽聽聽聽聽聽聽聽Current AHA Healthcare Provider training (BLS)
路聽聽聽聽聽聽聽聽Previous experience in patient care setting preferred.
路聽聽聽聽聽聽聽聽Prefer medical/surgical clinical experience
Licensure & Certifications Requirements
Must maintain BLS
Pay Range:聽$43.09-$79.71
