Under general supervision of a Pharmacy Supervisor is responsible for the interpretation, evaluation, preparation and dispensing of medications pursuant to a legitimate order in keeping with established policies, procedures, and standards of practice and scope of pharmacy services of the respective affiliate. Collaborates with multidisciplinary care providers to monitor, assess, and intervene as needed to ensure safe, appropriate, and cost-effective medication use. Oversees and closely reviews the work of pharmacy technicians and ancillary support personnel. Reviews and interprets each order to determine completeness, therapeutic appropriateness as it relates to the patient’s clinical status, concomitant drug therapy and diet, and with consideration of the aging processes and human developmental stages (neonate, child, adolescent, adult, geriatric). Contacts the prescriber, or covering prescriber, as necessary to initiate appropriate interventions to ensure therapeutic efficacy, minimize potential risks of drug therapy, and optimize cost effectiveness.Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications for the patient populations served.Provide drug information and consultation to prescribers, nurses, and other health care professionals regarding appropriate drug therapy, including drug interactions, side effects, incompatibilities, allergies, medication order discrepancies, drug stability problems, and drug administration. Proactively reviews and approves medication orders in LifeChart including drug, dose, dose form, frequency, and duration of therapy. Ensures relevant patient specific parameters are present in LifeChart and considered (i.e., diagnosis, known allergies, weight, age, etc.). Supervises work performed by pharmacy interns and technicians to ensure performance standards are maintained and work is in accordance with federal, state, and organizational rules and regulations.Identifies, documents, resolves, monitors, and prevents drug therapy related problems, including contraindications, duplications of therapy, and drug interactions.Engages with other clinical pharmacists or clinical pharmacist specialists for complex clinical situationsIntegrates patient-focused medication therapy management and pharmaceutical care in accordance with established clinical pathways or guidelines.Participates in prospective and retrospective pharmacokinetic dosing and monitoring of selected medications and recommends dose adjustments and laboratory monitoring as required.
