The South Provo Clinic provides a strong case management focus, helping clients navigate daily living needs, access community resources, coordinate medical care, and work toward stable housing. You'll collaborate with therapists, nurses, prescribers, housing authorities, and other community partners to help clients build greater independence and stability.
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Compensation: $21.36–$29.88 DOE and Licensure

What You'll Do as a Case Manager
- Conduct client assessments and gather essential background information.
- Develop individualized care plans based on client needs and treatment goals.
- Provide ongoing support, guidance, advocacy, and resources to help clients achieve greater stability and independence.
- Help clients access community resources, housing assistance, benefits, and healthcare services.
- Assist with daily living needs, including coordinating medical, dental, and vision appointments.
- Coordinate care with clinicians, therapists, nurses, prescribers, and other healthcare providers.
- Build strong rapport with clients while providing consistent, compassionate support.
- Maintain accurate documentation, case notes, and client records.
- Collaborate with the onsite team and community partners to support clients and clinicians.
Qualifications
Applicants must meet ONE of the following:
Note: Applicants under the "working toward SSW" pathway must obtain their SSW license within 24 months of hire.
- Associate's degree or equivalent and working toward SSW licensure
- Bachelor's degree with Social Service Worker (SSW) license
- Bachelor's degree in any field with willingness to pursue SSW licensure
Additional Requirements:
- Valid driver's license
- Basic understanding of Social Work principles
- Familiarity with social welfare policy and human development
- Ability to problem-solve and maintain a client-centered approach
- Spanish speakers may qualify for increased wages after testing
