The Outreach department within CNHHS currently provides services to clients enrolled in the ADvantage Waiver program for nearly 40 counties in Eastern Oklahoma. The ADvantage Case Manager at Cherokee Nation Home Health Service is responsible for overseeing and coordinating care plans for clients enrolled in the ADvantage Waiver program. This role involves assessing client needs, developing personalized service plans, and ensuring the delivery of high-quality home health services to promote client independence and well-being. Key Responsibilities: - Conduct comprehensive assessments of clients' needs and circumstances to develop individualized care plans. - Coordinate with healthcare providers, family members, and other stakeholders to ensure seamless delivery of services. - Monitor and evaluate the effectiveness of care plans, making necessary adjustments to meet clients' evolving needs. - Advocate for clients' rights and ensure their access to necessary resources and services. - Maintain detailed and accurate documentation of client interactions, assessments, and service plans. - Provide guidance and support to clients and their families, helping them navigate the healthcare system. - Ensure compliance with all relevant regulations and standards related to the ADvantage Waiver program. - Participate in ongoing training and professional development to stay updated on best practices in case management and home health services. QualificationsEDUCATION: Bachelors Degree in Social Work, Nursing, or a related field OKDHS ADvantage Case Management Certification, preferred EXPERIENCE: Minimum of one year of paid professional or technical experience providing direct care services to the aging or disabled population, required. SKILLS AND ABILITIES: Excellent communication skills, both written and verbal, for effective interaction with clients, families, and healthcare providers. Strong organizational skills with the ability to manage multiple cases simultaneously. Proficiency in using case management software and electronic health records. Ability to work independently and collaboratively with a team environment. Demonstrated problem-solving skills and the ability to make sound decisions in complex situations. Empathy and compassion for individuals from various backgrounds and with varying needs.
