Status: Full-Time
Offered Wage Range: $21.65- $23.42 per/hr.
Full Wage Range: $21.65 - $57.73 per/hr. (Represents the low and high end of the total salary range for this position.

Position Summary
Provides in-person and phone assistance with enrollments into the WA Cares, Community Living Connections, and other benefits identified by our funders and/or available programs as applicable. Participate in direct community and client support and activities related to WA Cares and Community Living Connections by providing assessments, referrals, and follow-up.

Essential Job Functions
· Client assistance through telephone, walk-in or email for intake, providing navigation assistance.
· Generating client account, completing screening for program eligibility, performing assessments and applications, assistance with platform setup.
· Schedule and perform in-home assessment, following safety protocols.
· Refers to provider network options for service delivery.
· Ensures all client records are accurately maintained in GetCare, by following the documentation policies and network standards in real time to document client process information and referrals given.

Requirements
Minimum Qualifications
Education & Experience
Four years of paid, full-time, on-the-job social service experience in providing case management or long-term support and services to vulnerable adults, such as providing one-on-one, in-person customer service, developing care plans for aged, blind, or disabled individuals and caseloads, and a high school diploma or its equivalent
Or
Two years of paid, full-time, on-the-job social service experience and at least two years of
college-level courses in a relevant field.
Or
One year of paid, full-time, on-the-job social service experience and a Bachelor’s degree in
social services, behavioral or health sciences.
Required Certifications & Licenses
• Valid driver’s license in state of residence.
• Auto insurance in the amount required by the State of Washington.
• Successful completion of a criminal history background check.
