Responsibilities for a RN Case Manager:
- Â Lead, inspire and motivate others to provide exceptional care.
- Â Visit our Pediatric/Adult clients and conduct assessments in Wyomissing, PA and other surrounding areas.
- Â Develop the care plan and oversee its execution.
- Â Supervise LPN's, RN's, and home health aides (HHA) staff in the field.
- Â Facilitate new hire orientation.
- Â Conduct competencies on new and tenured staff.
Qualifications for a RN Case Manager:
- Registered Nurse (RN) with 1 or more years of experience as an RN.
- Supervisory experience preferred.Â
