The Admissions Coordinator position is a full-time hybrid position during the day shift from 8:00am-5:00pm with rotating weekends.
Requirements of the Admissions Coordinator:
路 Possession of a current license to practice as a vocational nurse in California required.
路 One-year recent acute care experience; hospice experience preferred.
路 At least one year of experience demonstrating excellent customer service and communication skills.
路 Proficient in EMR systems, internet navigation, and data entry. Strong working knowledge of Microsoft Office or Google Suite, along with excellent data entry skills preferred.
路 Previous experience in the Admissions department, including expertise in Managed Care Insurance eligibility, verification, and authorization processes preferred.
路 Working knowledge of medical terminology.
Admissions Coordinator Duties Include: 聽聽
路 Assures that all requests for Hospice 聽聽services are responded to in a timely and professional manner and that all 聽聽referral sources are provided with options or creative solutions for patients 聽聽who present with unique or atypical needs.聽
路 Completes Eligibility and Insurance verification process in an accurate and timely manner.
路 Communicates with admission staff regarding initial patient needs, both verbally and in the medical record. Communicates new referrals to Hospice Team Assistants.
路 Participates in maintaining positive working relationships with contracted nursing facilities, hospitals, referring physicians, managed care organizations and private insurance companies.
路 Assists the Admissions Manager with assignment of each patient to a Nurse.
路 Participates in scheduled and ongoing meetings for improved communication and updates within the Agency.
路 Participates in the Agency Performance Improvement Program to identify areas of concern and to implement process improvements.
路 Assists Hospice Manager/Director in the maintenance of updated logs, worksheets, Agency files and calendars on a timely basis.
路 Answers incoming phone calls related to customer services and ensures consistent聽
路 follow-up.
