Nurse Practitioner  

Hourly Rate: $70.00 -$90.00/ Hr. 
Shifts Needed: Full time and part-time  Sunday-Saturday/hours are flexible 
Job Status: Part-time and Full-time positions available
Work Locations: Remote 

 

WHO WE ARE

Redwood Family Care Network – is enhancing access to care for the most vulnerable patients and communities. By partnering with Person Centered Health Services we are actively tearing down silos and bridging over barriers of access to care by leveraging collaborative solutions and emerging technologies to deliver consumer-directed, in-home services.

WHAT YOU’LL DO

The P.C.H.S Nurse Practitioner will deliver medical care to patients as part of a multidisciplinary care team. You will work with patients and caregivers in the home to coordinate care, educate on medical needs, and ensure patients’ social and clinical support for successful long-term health maintenance.

Responsibilities

  • Conduct telephonic and/or face-to-face outreach with patients to identify social determinants of health impacting patient's health and overall wellness
  • Physically examine patients in their home
  • Review diagnostic tests
  • Reconcile medications
  • Create chronic condition management care plans
  • Provide transitional care education
  • Collaborate with patients around their health care goals
  • Partner with RFCN enhanced care management team which includes physicians, nurses, pharmacists, therapists, social workers, community health workers, and medical assistants
  • Perform other related activities as assigned

WHAT YOU NEED

  • An advanced practice nursing degree from an accredited college or university
  • Nurse practitioner license in the State of California
  • Experience as a Nurse Practitioner/Registered Nurse in a clinic, hospital, ER, or other healthcare delivery setting
  • Certification in basic life support (BLS) required
  • Effective communication skills and experience working with other clinical providers, patients, families, and community resource organizations
  • Proficiency in primary care, acute care, and chronic care management practices
  • Knowledge of health and patient care regulations, and familiarity with value-based care delivery models

Education and Experience

  • Master's in Science of Nursing.
  • Nurse Practitioner license in the state of California.
  • Medical specialty in primary care and psychiatric.
  • 1 years experience as a Nurse Practitioner
