Job Summary and Responsibilities

As our Care Manager RN, you will be a dynamic leader, shaping clinical excellence and empowering your frontline nursing team to deliver unparalleled patient care.Every day, you will inspire and guide your department, translating organizational goals into action that elevates patient experiences, strengthens community ties, and ensures consistent delivery of quality care.   • Conducts in-depth assessments/reassessments covering medical, health, and rehabilitation concerns. Performs physical assessments as necessary and interpret clinical findings
  • Certifies level of care determinations
  • Works closely with physicians and other health care professionals. Consults with team members and participates in team care management activities
  • Develops care plans, implements and monitors services, consults with other team members, completes case recording and reporting. Maintains case load
  • Participates with CCT providing assessments, feedback and planning for patients
  • Assists with the development of organizational programs to create new services

 

Job Requirements

Required:

  • Three (3) years experience two (2) of which are in public health
  • Associate of Science degree in nursing
  • Valid current California license to practice nursing

 

Preferred:

  • Geriatrics experience 
  • Bachelors degree in nursing
