Overview
We are hiring a Full-Time Physical Therapist (PT) for a highly flexible Home Health position. This role offers one-on-one patient care, manageable territories, and strong earning potential—along with the ability to build your own schedule and work around your life.
Your Role:
As a Physical Therapist, you will evaluate, plan, and provide individualized treatment for patients in their homes while collaborating with a supportive clinical team.
What You’ll Do:
- Provide high-quality patient care using approved therapy techniques
- Evaluate patients and establish clear, measurable goals
- Develop and implement personalized treatment plans
- Supervise and collaborate with PTAs and HHAs
- Document patient progress accurately and on time
- Educate patients and families on exercises, mobility, and equipment use
- Assess home environments and recommend safety modifications
- Coordinate care with nursing, social work, and other clinical staff
- Maintain HIPAA compliance and patient confidentiality

Requirements:
- Bachelor’s, Master’s, or Doctorate degree in Physical Therapy
- Active Physical Therapy license in the state
- Valid driver’s license and reliable transportation
