Job Summary and Responsibilities

Position Summary:

Under direct supervision, the incumbent identifies and evaluates potential clients as total persons and identifies the functional limitations which impede independent living. This position reports to the RN Supervisor.

Job Requirements

Minimum Qualifications:

• Bachelors Other in social work from an accredited school
• Two years of experience providing social service in one or more of the fields of aging health and long term care services
• Basic Life Support - CPR (Must be through the American Heart Association)
