RN Home Health Case Manager

Position Responsibilities:

路聽聽聽聽聽聽聽聽聽Conduct comprehensive nursing assessments of patients' physical, emotional, and psychosocial needs.

路聽聽聽聽聽聽聽聽聽Develop, implement, and evaluate individualized care plans in coordination with the interdisciplinary team (including physicians, therapists, social workers, and aides).

路聽聽聽聽聽聽聽聽聽Provide direct skilled nursing care, such as wound care, medication management, IV therapy, symptom management (especially pain and comfort in hospice), and patient/caregiver education.

路聽聽聽聽聽聽聽聽聽Offer education and emotional support to patients and their families/caregivers based on individual needs.

路聽聽聽聽聽聽聽聽聽Supervise LPNs/LVNs, home health aides, and other support staff.

路聽聽聽聽聽聽聽聽聽Participate in on-call rotations, admissions, or urgent response as needed (common in hospice roles).

路聽聽聽聽聽聽聽聽聽Coordinate care activities, including visit scheduling and documentation to ensure compliance with regulations.

路聽聽聽聽聽聽聽聽聽Monitor patient progress, respond to changes in condition, and adjust care plans accordingly.

  • One (1) year experience as a RN.
  • Licensed to practice as a registered nurse.
  • Must possess and maintain valid CPR certification while employed in a clinical role.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
