About the Role:

As a Per-Diem Speech Therapist, you will play a vital role in helping patients regain their communication and swallowing abilities, improving their overall quality of life. You will provide one-on-one therapy in a home health setting, offering personalized treatment while collaborating with an interdisciplinary care team.

Key Responsibilities:

Conduct comprehensive assessments of speech, language, cognition, and swallowing abilities.
Develop and implement personalized treatment plans based on patient needs.
Provide skilled speech therapy services, addressing articulation, voice, fluency, comprehension, and swallowing disorders.
Educate patients and caregivers on speech exercises, communication techniques, and safe swallowing strategies.
Work closely with nurses, occupational therapists, and other healthcare professionals to ensure coordinated and holistic care.
Maintain accurate and timely documentation in compliance with regulatory standards.
Stay up to date with best practices and industry advancements in home health speech therapy.

Qualifications:

✔️ Valid State License as a Speech Therapist
✔️ Master’s Degree in Speech-Language Pathology
✔️ Minimum 1 Year of Experience in home health or acute care settings
✔️ Strong clinical skills with a patient-first mindset
✔️ Excellent communication & interpersonal abilities
✔️ Ability to work independently and as part of a collaborative team
✔️ Understanding of Medicare home health regulations & documentation
