The Physical Therapist PRN at our Reno Home Health Branch plays a critical role in delivering high-quality rehabilitative care to patients in their homes. This position focuses on assessing, planning, and implementing individualized physical therapy treatment plans to improve patient mobility, reduce pain, and enhance overall functional independence. The therapist will collaborate closely with interdisciplinary teams, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care. Flexibility and adaptability are essential, as this PRN role requires managing fluctuating caseloads and responding promptly to patient needs. Ultimately, the goal is to support patients in achieving optimal recovery outcomes while promoting safety and wellness in the home environment.
As a Physical Therapist, you will:
Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
Applies concepts of infection control/universal precautions in performing patient care
Assure continuity of quality patient care delivered with appropriate documentation
Provide effective communication to patient/family, team members and other health care professionals
Promote agency philosophy and administrative policies
Maintain and complete all documentation within company stated guidelines.
Requirements
Successful completion of accredited physical therapy program and state licensure exam.
Valid Physical Therapy license
1 year of home health experience preferred
CPR certification
Valid Driver’s license
Valid Auto Insurance
HomeCare HomeBase experience preferred
Physical Demands
Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
Must be able to lift, carry, pull, or push up to 100 lbs..
