Position Summary
We are seeking a dynamic and experienced Rehabilitation Manager (PT or OT) to lead our therapy team and oversee daily rehabilitation services. This role combines clinical expertise with operational leadership to ensure high-quality patient care, staff development, regulatory compliance, and program growth.
The ideal candidate is a motivated clinician with strong leadership skills, a passion for patient outcomes, and the ability to foster a collaborative, supportive team environment.
 
What You'll Do
Lead the Therapy Team
  • Provide day-to-day direction and support to therapy staff
  • Help create schedules and manage caseloads
  • Mentor clinicians and support professional growth
  • Assist with hiring, onboarding, and performance reviews
Support Great Patient Care
  • Ensure residents receive high-quality, person-centered therapy
  • Jump in to treat patients as needed
  • Help manage complex cases and discharge planning
  • Monitor outcomes and adjust programs for success
Keep Things Running Smoothly
  • Oversee documentation and regulatory compliance (Medicare/PDPM)
  • Track productivity and department performance
  • Coordinate with nursing and administration
  • Prepare for surveys and audits
Grow the Program
  • Look for ways to improve services and resident outcomes
  • Support census growth and community relationships
  • Promote a team culture that's positive, respectful, and collaborative
What We're Looking For
Required
  • Licensed Physical Therapist (PT) or Occupational Therapist (OT)
  • Graduate of an accredited program
  • At least 3 years of clinical experience
  • Strong organizational and communication skills
  • Passion for working with older adults
Preferred
  • Previous SNF experience
  • Prior leadership or supervisory experience
  • Knowledge of PDPM and skilled nursing regulations
  • Experience with electronic medical records
