Position Overview

Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate and motivated Home Health Physical Therapist (PT) to join our growing team serving Fort Worth, TX, Arlington, Mansfield, Burleson, Keller and surrounding communities. The Physical Therapist will provide skilled physical therapy services to patients in their homes, focusing on restoring mobility, reducing pain, improving strength, and helping patients regain independence following illness, injury, or surgery.

This role requires strong clinical judgment, excellent communication skills, and the ability to work independently while collaborating with an interdisciplinary healthcare team.

Key Responsibilities

  • Evaluate and assess patients’ physical condition, mobility limitations, and rehabilitation needs in the home setting.

  • Develop and implement individualized physical therapy care plans based on physician orders and patient goals.

  • Provide therapeutic interventions including exercise therapy, gait training, balance training, mobility training, and manual therapy.

  • Educate patients and caregivers on exercise programs, fall prevention strategies, safety techniques, and proper use of assistive devices.

  • Monitor patient progress and modify treatment plans as needed to ensure optimal outcomes.

  • Document patient evaluations, treatments, and progress notes accurately and in a timely manner in the electronic medical record system.

  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to coordinate patient care.

  • Participate in case conferences and interdisciplinary care planning meetings when required.

  • Ensure compliance with Medicare, state, and agency home health regulations.

  • Communicate regularly with physicians regarding patient progress, changes in condition, and discharge planning.

  • Recommend adaptive equipment or home modifications to improve patient safety and independence.

  • Maintain productivity standards and complete documentation within agency timelines.

Qualifications

  • Current Texas Physical Therapist (PT) license in good standing.

  • Graduate of an accredited Physical Therapy program (DPT or equivalent).

  • Minimum 1 year of experience preferred; home health experience is a Plus.

  • Current BLS/CPR certification.

  • Valid Texas driver’s license, reliable transportation, and current auto insurance.

  • Ability to work independently and manage a flexible schedule.

  • Strong clinical assessment, communication, and documentation skills.

Preferred Skills

  • Experience with home health EMR systems such as WellSky (Kinnser) or Homecare Homebase.

  • Knowledge of OASIS documentation and Medicare home health guidelines is a Plus.

  • Experience working with geriatric and post-acute care patients.

  • Strong patient education and interdisciplinary collaboration skills.
