Job Description Summary: The Hospice Case Associate (RN or LPN) is responsible for providing direct patient care. Responsibilities include following the plan of care, providing treatments, working collaboratively with the team to meet positive patient care outcomes, and performing case management duties. 

Essential Job Functions/Responsibilities:

  1. Provides direct patient care as defined by the State of Washington.
  2. Implements current nursing practice following a comprehensive assessment and the plan of care.
  3. Provides accurate and timely documentation of patient services to reflect the plan of care.
  4. Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and self-care.
  5. Participates in the coordination of hospice services, appropriately reporting identified needs to the interdisciplinary group. Examples include hospice aide, OT, PT, MSW, ST, Dietitian, or Clinical Manager.
  6. Uses equipment and supplies effectively and efficiently.
  7. Provides appropriate pain/symptom management. Evaluate the patient’s response to treatments/medications.
  8. Participates in personal and professional growth and development. Also participates in quality assessment performance improvement teams and activities.
  9. Performs case management duties, including coordinating care plans, facilitating communication among the care team, and ensuring continuity of care.
  10. Assists with discharge planning and follow-up care.
  11. Performs other duties as assigned by the registered nurse.

Position Qualifications:

  1. Registered nurse or licensed practical nurse with current licensure to practice nursing in the state of Washington.
  2. Community health/hospice or medical/surgical experience is preferred.
  3. Complies with accepted professional standards and practice.
  4. Demonstrates good verbal and written communication, and organizational skills.
  5. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  6. Possesses and maintains current CPR certification.
  7. Understands the philosophy of the hospice concept and the needs of the terminally ill.
Benefits
  • Full-time employees accrue up to 168 hours of PTO annually, in addition to accruing sick time at the rate of 1 hour for every 40 hours worked. Bereavement and jury duty leave also available. 
  • Option to enroll in medical, dental, vision, and life insurance. Heartlinks pays for 100% of the employee's medical premiums, and 75% of the employee's dental and vision premium. Health benefits begin the first of the month after start date. 
  • Miles driven for work are reimbursed at per mile.
  • Holiday pay for 6 observed holidays.
  • Ability to enroll in 401(k) and begin contributing at time of hire. Heartlinks will match dollar for dollar up to 5% of earnings after one year of employment and 1080 hours worked.

Schedule: 
Wednesday - Sunday, 8hrs per day.
