Position: Licensed Practical Nurse (LPN)

About the Role:

The Orthopedic Clinic is seeking an experienced and compassionate Licensed Practical Nurse (LPN) to join our growing team in the Bone Health Program.

This specialized role will primarily support the Fracture Prevention Program, working closely with the Bone Health APP, Clinic Manager, and Assistant Clinical Manager. As an integral part of our team, you'll provide consistent and continuous care to patients across multiple sites in the Richmond area.

Key Responsibilities:

• Support and travel with the provider to local sites, ensuring continuity of care for patients in the Fracture Prevention Program.
• Deliver and document direct, age-specific care based on medical orders and an integrated, multidisciplinary plan.
• Administer prescribed medications safely, identifying any incompatibilities or interactions.
• Provide direction and education to patients, families, and healthcare team members.
• Assist in the prevention of illness and injury, maintenance of wellness, and provision of comfort.
• Collaborate with the healthcare team to develop and implement patient care plans.
• Begin assuming leadership responsibilities at the unit level as your role grows.

Qualifications:

• Minimum 5 years of LPN experience required.
• Orthopedic/Fracture Prevention/Osteoporosis experience preferred, but not required.
• Strong critical thinking skills and the ability to work independently.
• Excellent organizational skills and a keen attention to detail.
• High-level communication skills with the ability to interact effectively with patients and providers.
• A passion for working in a specialized, patient-centered environment.

Experience PREFERRED:Experience as a LPN in either an acute care setting or as applicable to area of practice

Education/training REQUIRED:Graduate from a Board of Nursing approved School of Practical Nursing

Education/training PREFERRED: N/A

Independent action(s) required:Follows documented physician/licensed independent provider orders, standards, and practice guidelines such as treatments and organizes work to carry out assignment.

Supervisory responsibilities (if applicable): N/A
