About the Role: The Advanced Practice Provider (APP) plays a critical role in delivering high-quality, patient-centered care across a diverse range of medical conditions. This position involves collaborating closely with physicians, nurses, and other healthcare professionals to assess, diagnose, and manage acute and chronic illnesses. The APP will be responsible for developing and implementing individualized treatment plans, performing procedures within scope of practice, and ensuring continuity of care through effective patient education and follow-up. This role demands a strong commitment to evidence-based practice, patient safety, and continuous professional development to improve clinical outcomes. Ultimately, the APP contributes significantly to the efficiency and effectiveness of the healthcare team, enhancing patient satisfaction and overall health system performance.

Minimum Qualifications:

  • Master’s degree or higher in Nursing (MSN, DNP) or Physician Assistant Studies from an accredited program.
  • Current and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in Virginia.
  • Minimum of 2 years clinical experience in a relevant setting.

Preferred Qualifications:

  • Experience working in a multidisciplinary teams and community health setting.
  • Proficiency in multiple languages to effectively communicate with diverse patient populations.
  • Familiarity with electronic health record (EHR) systems and telemedicine platforms.
  • Strong background in patient education and chronic disease management.

Responsibilities:

  • Conduct comprehensive patient assessments including history taking, physical examinations, and diagnostic evaluations.
  • Develop, implement, and modify treatment plans in collaboration with the healthcare team.
  • Order, interpret, and follow up on diagnostic tests and laboratory results to guide patient care decisions.
  • Perform procedures and interventions within the scope of practice and institutional protocols.
  • Provide patient education on disease management, medication adherence, and preventive health measures.
  • Document all patient encounters accurately and timely in the electronic health record system.
  • Participate in multidisciplinary team meetings to coordinate care and optimize patient outcomes.
