VCU Health is seeking an authentic, passionate and inspiring nurse Manager to help lead our virtually enabled Hospital-at-Home (HaH) program. Housed in the health system’s Continuum Integration Center (CIC) in north Richmond, this exciting opportunity offers innovation, professional growth, and opportunity to be a part of an already incredible team. HaH care is delivered via a mix of in-person nursing and paramedic visits, virtual nursing and vital sign monitoring, and telehealth physician visits. The nurse Manager supervises staff whose roles are in the home (visiting RNs), in VCU Medical Center (patient acquisition RNs), and in the CIC command center environment (monitoring RNs). The nurse Manager position supports the continued growth and development of a robust VCU Health at Home strategy, and reports directly to the Nursing Director, VCU Health at Home | Continuum Integration | Senior Services. Hospital-at-Home programs are gaining momentum nationwide, because of the benefits for patients and health systems, including increased patient satisfaction, improved patient outcomes, better performance on value-based metrics and relieving facility capacity constraints.
The focus of the position:
VCU Health at Home – Inpatient nurse Manager oversees the 24/7 Hospital at Home ā€œvirtual unitā€ - which includes hiring, training, managing multi-disciplinary team members, creating and implementing business strategies, and implementing nursing practice tools and digital health resources to support a care delivery system that produces quality care including decanting VCUMC low acuity patients to create high acuity capacity as well as readmission avoidance. The role collaborates closely with Nursing Director to align clinical operations with organizational goals, optimize operations and workflows, drive quality and safety initiatives, and enhance both patient experience and team member engagement to achieve high-performance programs delivering quality outcomes in this specialized patient population. The nurse Manager oversees all daily operations, staffing, technology, and regulatory compliance across assigned inpatient programs and serves as a primary liaison and information resource for the service unit while overseeing the provision of therapeutic care in a holistic and evidence-based environment which consistently supports VCU Health System's mission and strategic plans.

The Manager VCU Health at Home provides oversight and management to inpatient and/or outpatient programs within the Continuum Integration Center (CIC), including hiring, training, daily operations, staffing, technology, and regulatory compliance to support high‑quality care, readmission avoidance, ED decompression, and hospital throughput. The role partners closely with department leadership to align operations with organizational goals, optimize workflows, and drive quality, safety, patient experience, and team engagement. This leader serves as the primary liaison for assigned programs and ensures holistic, evidence‑based care consistent with VCU Health’s mission.

Essential Job Statements

Leadership -Ā FunctionsĀ as the primary clinical administrative leader forĀ assigned CIC units or programsĀ servingĀ as a shared service supporting all VCUHS hospitalsĀ (includingĀ CHoR),Ā andĀ functions as a key CIC leadership team member to accomplish goals and objectives of theĀ CICĀ and the organization, upholding quality management, patient care, and departmental standards; assists in developing policies, procedures, and standards that reflect evidence‑based practice.Ā 

Management – LeadsĀ team(s)Ā with superior interpersonal and communication skills sufficient to effectively create relationships to support a healing environment with patients, families, staff,Ā providers, and administrative personnel including senior management in both written and verbal communication; continually demonstrates and supports a Professional Practice model that allows nurses to work in collaboration with interdisciplinary partners to achieve high quality patient outcomes.Ā 

Human Resources – HoldsĀ responsibilityĀ forĀ the recruitment and retention of qualified staff and to ensure appropriate orientation, education, and continuing professional development of personnel through mutualĀ goal setting and performance evaluation;Ā coaches and counsels to correct identified performance deficiencies.Ā 

Operations - Responsible for the coordination, organization and delegation of operational aspects of the CIC’s practices/programs to ensure effective, efficient delivery of services;Ā provides oversight and supervision to the overall practice environment assisting all other disciplines and directing clinical/administrative activityĀ andĀ serves as part of nursing on-call coverage.Ā 

Budget andĀ FinanceĀ -Ā Responsible for budgetary oversight, and along with staff involvement, accountable to the organization for maintaining aĀ cost-effectiveĀ unit/program; monitors utilization of equipment and supplies and prioritizes budget requests to ensure safeĀ patientĀ care and optimal utilization of resources.Ā 

Performance Improvement – Leads and participatesĀ inĀ performance improvement activities to improve quality of nursing care, promote patient outcomes and provide a cost-effective environment for patient care; works closely with staff and peers to identify opportunities to improveĀ patientĀ careĀ and resolves patient care issues as identified.Ā 

CareĀ Experience -Ā Creates a caring and compassionate patient focused experience by building healing relationships with patients, families, and colleagues, and serves as the departmental role model in all aspects of patient andĀ home/post-acute/continuum/digitalĀ care; demonstrates and promotes a high regard for the dignity, respect and safety of all patients, family members,Ā providersĀ and personnel.Ā 

Change Management - Orchestrates complex change and acknowledges the psychological transition on self and others; involves stakeholders and experts in planning, designing and redesigning change and uses appropriate interpersonal styles and techniques to gain acceptance of ideas or plans; supports staff during times of difficult transitions.Ā 

Innovation - Collaborates with Associate Director inĀ developingĀ and implementing new and/or innovative operational services that meet the evolving needs of the health system and its patients; aids in developmentĀ ofĀ materialsĀ to provide educationĀ and marketing toĀ enhanceĀ referrals as well asĀ clinician/patient/providerĀ awareness.Ā 

Technology Advancement - Actively monitors emerging digital health trends, new technologies, and market developments with a focus on identifying, evaluating, and implementing emerging tools as applicable to improveĀ patientĀ care and operational efficiency; leverages comprehensive suite of Epic EMR tools designed to streamline documentation, integrate digital health roles, and enhance clinical workflows including AI-driven features.Ā 

Miscellaneous -Ā Performs other duties as assigned and/or participates in special projectsĀ toĀ support the mission of VCUHS and department.Ā Provides assistance toĀ team members. Accepts alternate assignments as required.

Employment QualificationsĀ 

YearsĀ and TypeĀ ofĀ RequiredĀ Experience: Five (5)Ā years of relevant clinical experience withĀ two (2)Ā of those yearsĀ ofĀ progressive leadership and/or supervisory/management experienceĀ 

Years and Type ofĀ PreferredĀ Experience: Eight (8)Ā years of relevant clinical experience withĀ five (5)Ā of those years of progressive leadership and/or supervisory/management experienceĀ in home care,Ā preferred. Experience in operations of Remote Patient MonitoringĀ providingĀ post-acute care orĀ caring forĀ chronic conditions in the home setting,Ā preferred. Experience inĀ home care,Ā post-acute care,Ā academic medical center,Ā and/or health insurance authorization,Ā preferredĀ
