The VA - Nurse Manager plays a critical role in ensuring high-quality clinical services are delivered efficiently and effectively across home-based and mobile care environments.
Position Overview
The VA - Nurse Manager provides leadership, oversight, and support to a team of traveling Registered Nurses delivering care in patient homes and community-based settings. This role ensures clinical excellence, operational efficiency, and compassionate patient-centered care aligned with Ponos Care’s mission and clinical standards.
The VA - Nurse Manager collaborates closely with clinical leadership to ensure nursing staff are supported, care delivery is coordinated effectively, and patient outcomes reflect the highest standards of quality and safety. This role also ensures compliance with regulatory requirements and promotes continuous improvement in clinical practice.
Core Responsibilities
1. Clinical Operations Leadership
- Manage daily home-based clinical service delivery across RN teams
- Coordinate schedules, assignments, and patient visits to optimize care delivery
- Ensure timely and accurate EMR documentation and workflow execution
- Implement care protocols and support treatment plan adherence
2. Patient Care Quality & Outcomes
- Ensure delivery of high-quality, patient-centered, culturally competent care
- Monitor care plans and support complex case management
- Improve:
- Patient satisfaction and experience
- Continuity of care and engagement
- Reduction in missed visits and readmissions
- Strengthening care coordination across interdisciplinary teams
3. Care Coordination & Scheduling Management
- Oversee home visit scheduling across multiple regions
- Ensure appropriate staffing coverage for patient needs
- Optimize nurse utilization and reduce scheduling gaps
- Address real-time scheduling challenges and adjust resources
4. Regulatory & Clinical Compliance
- Ensure compliance with clinical standards, HIPAA, and documentation requirements
- Monitor adherence to scope of practice, licensure, and certifications
- Maintain audit readiness and escalate risks as needed
- Enforce policies, procedures, and quality standards
5. Team Leadership & Workforce Management
- Supervise, coach, and mentor RN staff
- Conduct performance evaluations and ongoing feedback
- Support hiring, onboarding, and performance management
- Improve nurse engagement and retention (target: 90–95%)
- Foster a culture of accountability, teamwork, and continuous improvement
6. Collaboration & Communication
- Partner with Director of Clinical Services and leadership team
- Communicate operational performance, risks, and improvements
- Collaborating with interdisciplinary teams to improve care delivery
- Promote a culture of professionalism, inclusion, and patient-centered care
7. Operational Excellence & Program Support
- Improve efficiency of clinical workflows and care coordination processes
- Monitor productivity, workload balance, and service utilization
- Support implementation of telehealth and new care delivery models
- Identify opportunities to improve patient care and operational performance
QUALIFICATIONS AND EDUCATION
- Bachelor’s degree in nursing or related field
- Active Registered Nurse (RN) license in applicable state(s)
- Current certifications such as BLS, ACLS, CPR (as applicable)
- 3+ years clinical experience in acute care, infusion therapy, pain management, hematology, or community health
- 1+ years of nursing leadership or supervisory experience
- Experience coordinating care in home health or mobile healthcare environments preferred
- Ability to travel to patient locations as needed
