The Care Coordination Assistant – NFAS provides administrative, scheduling, and care coordination support for patients receiving care in skilled nursing and long-term care facilities. This role coordinates patient admissions, appointments, referrals, medical records, insurance verification, and provider communications to support continuity of care.
Specific Responsibilities include coordinating onboarding activities for newly admitted NFAS patients; scheduling, rescheduling, and managing appointments and referrals; communicating appointment reminders and updates to facility contacts; processing medical records and provider documentation through electronic fax and record management systems; obtaining records from external facilities; monitoring work queues, secure messaging, and email communications; performing insurance eligibility verification; supporting provider credentialing activities; monitoring office supply inventory; and assisting with departmental operations, special projects, and cross-coverage needs. The Care Coordination Assistant serves as a key liaison between providers, facility staff, and internal teams, ensuring efficient workflows and high-quality, patient-centered care.

Experience REQUIRED:
Minimum of two (2) years of healthcare related experience
Knowledge of computers and various computer programs such as Microsoft Suite products, [especially, Word and Excel] and Adobe Professional
Experience PREFERRED:
Three to five (3-5) years experience in acute care academic center or comparable organization
Education/training REQUIRED:
High School Diploma or equivalent
Education/training PREFERRED:
Some college level education
Independent action(s) required:
Coordinates the communication and referrals to post discharge providers including home health, home and inpatient hospice, Long Term Acute Care facilities, Skilled Nursing facilities, Assisted Living facilities, DME, transportation and any other type of post discharge provider of services required.

Compensation Range
$16.08 - $24.12Actual salary offers will be based on several key factors to include relevant work experience, credentials, and qualifications.
