Position: Continuum Integration Center Enrollment LPN

Shift: 10hrs shifts 8am – 6pm with a Holiday and weekend rotation

VCU Health is seeking authentic, passionate and inspiring candidates for our virtually enabled Hospital at Home and Transitional Remote Patient Monitoring (tRPM) program.  This exciting opportunity offers innovation, professional growth, and opportunity to be a part of an already incredible team. 

The Enrollment LPN ensures quality successful enrollment of patients into the tRPM program.  This includes management of a referral work queue, bedside education on the tRPM program and equipment with patients prior to hospital discharge, and enrolling patients in a the tRPM technology platform.

Transitional Remote Patient Monitoring a 30-day post discharge program that supports patients utilizing vital sign monitoring, nurse triage, provider telehealth visits, and nurse navigation.

The LPN job commences to integrate technical skills and theoretical knowledge to meet the needs of the patient and family. This role assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort. This role also provides direction, education, and begins to assume leadership responsibilities at the unit level.

The LPN job delivers and accurately documents direct, age-specific care to a patient or group of patients based on medical orders and an integrated, multidisciplinary plan of care. This role safely administers prescribed medications, identifying incompatibilities and/or interactions."

Licensure, Certification, or Registration Requirements for Hire:
Licensure, certification or registration varies depending on the incumbent`s employment background and transitional assignment;
LPN licensure in Virginia or eligible

Licensure, Certification, or Registration Requirements for continued employment:
Current LPN Licensure in Virginia AHA BLS Certification or equivalent

Experience REQUIRED:
Varies based on transitional duty assignment.

Experience PREFERRED:
Experience as a LPN in either an acute care setting or as applicable to area of practice

Education/training REQUIRED:
Graduate from a Board of Nursing approved School of Practical Nursing

Education/training PREFERRED: N/A

Independent action(s) required:
Follows documented physician/licensed independent provider orders, standards, and practice guidelines such as treatments and organizes work to carry out assignment.

Supervisory responsibilities (if applicable): N/A

Additional position requirements:
Licensed Practical Nurses work all three shifts, weekends and holidays.
Individual shift rotation and schedules will be established with your specific Nurse Manager/Practice Manager.
Schedules, including requirements of shift rotations and hours of work may be adjusted as necessary to meet unit requirements and/or as needed.
