We are seeking an experienced and compassionate Director of Rehabilitation to join a Skilled Nursing Facility in Concord, California. This leadership role is responsible for overseeing rehabilitation services, supporting therapy staff, ensuring quality patient care, and driving positive clinical outcomes within a geriatric and post-acute care environment. The Director of Rehabilitation will work closely with interdisciplinary teams to maintain operational efficiency, regulatory compliance, and high standards of patient-centered care. Requirements

• Oversee daily rehabilitation operations within a Skilled Nursing Facility setting
• Supervise and support therapy staff including PTs, PTAs, OTs, COTAs, and SLPs
• Ensure delivery of high-quality patient-centered care and compliance with facility standards
• Coordinate staffing, scheduling, productivity, and program development initiatives
• Collaborate with interdisciplinary healthcare teams to support patient recovery and discharge planning
• Monitor clinical documentation, patient outcomes, and regulatory compliance
• Provide mentorship, training, and performance support for rehabilitation staff
• Assist with operational planning, quality improvement initiatives, and department growth
