Overview

At FutureCare our Admissions Coordinators oversee all aspects of the admissions process for our residents. Under the direction of the Admissions Director, the Admissions Coordinator assist with completing all necessary documents for resident admission to include the contract, arbitration agreement, Assignment of Benefits (AOB), Medicare Secondary Payer (MSP), and other admission paperwork as deemed appropriate.

Salary Disclosure Statement

The salary mentioned above reflects the potential base pay range for this role. Bonuses or other incentives (if applicable) are offered separately and paid pursuant to the relevant program schedule. All employment offers will consider such factors as overall experience, job-related qualifications, location, certifications/training, etc.

Responsibilities

  • Assist in managing the electronic referral systems for all supporting hospitals
  • Work with our Regional Care Manager’s, Social Workers, Case Managers, referring physicians, agencies, families, interested parties or legal representatives to gather medical, social and financial information pertaining to prospective clients
  • Coordinate with the Regional Care Manager for clinical screen through (ACTFast) software and necessary supporting documentation and obtain clinical approval from nursing designee when not an ACTFast clinical screen
  • Establish and verify payment source of the prospective client
  • Conduct facility tours and completing the admissions process with respective families/residents, as outlined in Admissions Policies and Procedures
  • Coordinate admission with all parties, i.e., referring hospital, family and facility departments to ensure a smooth transition for resident
  • Spot check available rooms for admission readiness
  • Assist Admissions Director with all pre-admission paperwork
  • Complete all necessary documents for admission and discuss all issues as outlined in Admission Policies and Procedures
  • Assist the Admissions Director with daily and monthly reports as assigned and required.
  • Will be required to work independently on Saturday as scheduled

Qualifications

  • High school diploma required
  • Bachelor’s degree preferred; or 5 years work experience in the healthcare field in lieu of a Bachelor’s degree
  • Excellent oral and written communication skills
  • Excellent organizational skills
  • The ability to work independently, as well as part of an interdisciplinary team of healthcare professionals
