Pay Range: $20.00 - $25.00 per hour, payable bi-weekly

JOB PURPOSE
Inland SoCal 211+ manages San Bernardino County’s Coordinated Entry System (CES), a streamlined,Â
evidence-based process that connects people experiencing homelessness to appropriate housingÂ
and services. Required for all HUD- and CoC-funded providers, CES ensures consistent assessmentsÂ
and a “no wrong door” approach, helping individuals and families access support through anyÂ
participating agency.
CES Outreach Coordinators answer live calls, conduct prescreening, provide in-person assistance, andÂ
work closely with the housing team to engage unhoused people in the CES process. This positionÂ
helps people enter and continue on pathways to permanent housing. CES Outreach CoordinatorsÂ
provide customer service with a trauma-informed, whole-person, evidence-based approach. ThisÂ
position is excellent for those who are skilled at providing compassionate care while adhering to aÂ
standard, consistent, and detail-oriented workflow and has an important role in helping communityÂ
members back into housing.
ESSENTIAL DUTIES
• Provide excellent customer service to individuals experiencing homelessness or a housingÂ
crisis, both over the phone and in person.Â
• Perform weekly outreach to provide in-person assistance to individuals experiencingÂ
homelessness in the community and distribute assistance such as bus passes and gas cards.Â
• Conduct case management for CES outreach clients, assess eligibility with prescreening andÂ
VI-SPDAT tools, and record client information in various systems such as but not limited toÂ
inContact, iCarol, HMIS, and Excel spreadsheets.Â
• Coordinate housing with clients, housing locators, and agency program staff, facilitate smoothÂ
transitions to other service and housing providers (warm handoffs), and collect follow-upÂ
information and document outcomes.Â
• Respond to crises and calls from stakeholders about individuals in need of assistanceÂ
• Provide clients with preliminary assistance such as becoming “document ready” for housingÂ
and addressing immediate needs such as food, clothing, hygiene, and other basic itemsÂ
• Identify available community resources and provide in-depth referrals to housing, medical,Â
and mental health services, while advocating on behalf of clients for services and housing.Â
• Develop relationships with external partners, coordinate outreach events; attend teamÂ
meetings, case conferences, training workshops, and community meetings; and documentÂ
cases through appropriate systems, ensuring compliance with CES policies and procedures.Â
• Work closely with engagement teams, case managers, and housing navigators, and provideÂ
Housing Navigation services on a case-by-case basis. • Abide by CES policies, procedures,Â
reporting, implementation, and other requirements.
• Successfully meet and maintain performance parameters and learn through various means toÂ
maintain appropriate knowledge of housing programs and services.Â

Requirements
QUALIFICATIONS
• Applicants should have the education, experience, and/or training necessary to fulfill the jobÂ
duties and responsibilities. Examples of how this requirement may be met include:
o High School Diploma, GED, or High School Equivalency with 2 years of experienceÂ
working with community members experiencing homelessness and/or performingÂ
outreach or direct services to a similar population.
• Ability to listen with sensitivity to other people’s feelings, needs, and points of view,Â
demonstrate respect, tact, and courtesy in expressing options or ideas.Â
• Passion and desire to work towards ending homelessness, patience to manage critical casesÂ
such as chronic disabilities, and ability to recognize and act upon opportunities to enhanceÂ
community relations.
• Excellent writing skills, including correct grammar and the ability to summarize, and proficientÂ
computer skills with competency in MS Office and various software.Â
• Preferred:
o English/Spanish bilingualÂ
o Proficiency in inContact, iCarol, and HMISÂ
o Knowledge of local resources and HUD regulationsÂ
o Mental health and substance use clinical skills
o Public speaking experienceÂ
• Ability and commitment to using a trauma-informed and equity lens and empatheticallyÂ
respond to the interconnectedness of barriers experienced by participants.
• Work Environment and Physical Requirements:
o In-Field Outreach involves working with individuals experiencing homelessness whoÂ
may be on the streets, in shelters, or in other places not meant for human habitation.Â
There may be integral challenges such as exposure to various types of infestationsÂ
and unhygienic situations due to homelessness or the mental health of the populationÂ
served. While ISC211+ makes every effort to protect all employees from unpleasantÂ
situations, the company is not responsible for resolving the effects of some of theÂ
work exposures since they are inherent in the environment. Personal protectiveÂ
equipment will be provided as required. The position requires frequent sitting, walking,
standing, crouching, kneeling, stooping, and lifting and carrying objects up to 25Â
pounds, and the ability to drive for various periods of time.
o Administrative Work is performed in an office using standard office equipment suchÂ
as the telephone, computer, fax machine, and copier. Tasks include moving objects like
laptops and chairs, repetitive wrist and hand motions, document preparation andÂ
analysis, transcribing information, navigating software, and prolonged sitting. TheseÂ
represent what is necessary to perform the essential duties. ReasonableÂ
accommodations for individuals with disabilities will be provided, as required byÂ
applicable law.
• Driving: A driver’s license, reliable transportation, and the minimum personal liabilityÂ
insurance as required by California law and ISC211+ insurance carriers are required. Mileage isÂ
reimbursed at IRS standard rate. This position involves driving a company vehicle and/orÂ
personal vehicle to various locations and encampments where clients reside, attendingÂ
meetings and events throughout San Bernardino County, regular off-site or field work,Â
transportation of equipment or materials, or time-sensitive responsibilities. Mileage inÂ
personal vehicles is reimbursed at IRS standard rate.
• Remote and Hybrid Work Requirements: For work performed remotely, employees mustÂ
have access to a private high-speed internet connection and private work area in which nonemployees cannot access or overhear confidential information. Remote work locations areÂ
subject to the same professional conduct and safety requirements as in-person work.

but not limited to active listening and the ability to adapt communication style to theÂ
audience (e.g., colleague, client, external, in-person, virtual, or written), as appropriate to role.

GRANT FUNDED POSITION
This position is grant funded through June 2026 and carries the possibility that it may not be renewedÂ
if further funding is not obtained. The availability of grant funds does not guarantee continuedÂ
employment.
32-HOUR WORKWEEK
Effective July 6th, 2025, we revised the standard workweek for all full-time employees to 32 hours in aÂ
6-month trial period. Continuation is contingent on the trial's success. This position's schedule and, ifÂ
it’s hourly, the pay rate, may revert to a 40-hour workweek on a company-wide or department basis.
