Heritage Health Network (HHN) is seeking a Licensed Vocational Nurse (LVN) to join our Enhanced Care Management (ECM) team supporting members within the IEHP Pod. The LVN will work closely with RNs, Lead Care Managers, Community Health Workers, Behavioral Health Care Managers, and the Director of Clinical Operations to provide coordinated, whole-person care.
This is a member-facing role that combines clinical support, care coordination, community-based member engagement, and clinical documentation.
What You’ll Do
- Provide clinical support and care coordination for an assigned caseload of ECM members within the IEHP Pod.
- Support individualized, person-centered care plans and address barriers to care, including avoidable ER and hospital utilization.
- Conduct member visits in the community or home and accompany members to medical appointments as needed.
- Complete risk assessments and collaborate with RNs and Lead Care Managers to develop and update care plans.
- Use motivational interviewing to engage members, encourage adherence, and support behavior change.
- Coordinate with hospitals and other providers on discharge planning and transitional care.
- Review clinical documentation and care plan notes for accuracy and completeness.
- Participate in daily pod huddles, case conferences, and multidisciplinary care reviews.
- Help members access appropriate healthcare services, resources, and community support.
- Maintain accurate, timely, and compliant documentation in ECW.
- Ensure documentation supports health plan, regulatory, billing, and payment requirements.
- Escalate cases outside the LVN scope of practice to the RN or Director of Clinical Operations.
Requirements
What We’re Looking For
- Active California LVN license in good standing.
- Completion of an accredited LVN program.
- 2+ years of nursing experience, preferably in care management, ambulatory care, ECM, managed care, or a related setting.
- Strong clinical assessment, care planning, and care coordination skills within LVN scope of practice.
- Knowledge of Medi-Cal, managed care, or care management practices.
- Strong communication and collaboration skills.
- Ability to work effectively within a multidisciplinary care team.
- Ability to conduct regular local travel for member visits.
- Bilingual English/Spanish is a plus.
- Experience with CalAIM, ECM, or managed care programs is preferred.
