Job Summary
The Physical Therapist (PT) is a qualified professional who directs, supervises, evaluates, and provides physical therapy services to patients/clients in the home as prescribed by the attending physician.

Qualifications

Graduate of an AMA‑approved (or equivalent) PT program
Licensed PT in the state of practice
2+ years experience preferred
1 year of Home Health experience required
Current CPR certification
Ability to work independently
Strong verbal and written communication skills
Valid driver’s license and insured vehicle
Meets pre‑employment health requirements

Responsibilities

Provide PT services as ordered by physician
Assist with patient evaluations and Plan of Care
Document and report patient progress
Educate patients, families, and staff
Evaluate home environment as needed
Maintain accurate clinical documentation

This position is based on a Pay per visit compensation model.

Pay per visit ranges from $100 to $150 visit commensurate with experience and depending on type of visit.
