Hospice RN Case Manager:
Job Summary
The Hospice Registered Nurse Case Manager participates as a member of the Interdisciplinary Team (IDT) to provide holistic care to patients and families. This role is responsible for comprehensive assessments, care coordination, and ongoing evaluation of patient and family needs. The RN ensures implementation and ongoing updates of the interdisciplinary plan of care to support comfort, dignity, and quality of life at end of life.
What you will do as a Hospice RN Case Manager:
- Provide case management for assigned hospice patients
- Conduct comprehensive assessments and ongoing visits
- Develop, implement, and update individualized Plans of Care with the IDT
- Manage pain and symptom control in collaboration with the physician
- Educate patients and families on medications, symptom management, and end-of-life care
- Coordinate care with physicians, interdisciplinary team members, and caregivers
- Provide skilled nursing interventions including wound care, IV therapy, and symptom management
- Participate in IDT meetings, on-call rotation, and quality improvement initiatives
Home Health RN Case Manager:
Job Summary
The Home Health RN Case Manager administers skilled nursing care to patients requiring intermittent professional services. This role focuses on coordinating, implementing, and evaluating care in the home setting in accordance with physician orders and individualized plans of care. The RN also provides patient and family education to promote independence, safety, and optimal health outcomes.
What you will do as a Home Health RN Case Manager:
- Conduct skilled nursing visits in patients’ homes
- Complete admissions, re-certifications, recertifications, and discharge assessments
- Develop, implement, and revise Plans of Care based on patient needs
- Complete OASIS documentation accurately and within required timeframes
- Coordinate care with physicians, therapists, and interdisciplinary team members
- Provide patient and caregiver education on disease process, medications, and self-care
- Monitor patient progress and adjust care plans as needed
- Participate in case conferences, on-call rotation, and administrative meetings
What we are looking for in a RN Case Manager (Both Roles):
- Graduate of an accredited nursing program
- Minimum of 2 years nursing experience (home health or hospice experience, acute care, or geriatrics preferred; OASIS experience strongly preferred (home health)
- Active Virginia RN license (or compact license)
- Current BLS/CPR certification or obtained within 30 days of hire
- Strong clinical assessment, communication, and decision-making skills
- Comfort with EMR systems and mobile documentation tools
- Valid driver’s license, reliable transportation, and proof of insurance
Schedule & Compensation:
Schedule: Monday–Friday, approximately 8:30 AM – 5:00 PM (with participation in on-call rotation as required)
Location: Roanoke and surrounding service areas
Starting Salary: $75,000/year
