Home Health Licensed Therapy Assistant (PTA):
The Licensed Therapy Assistant provides skilled care to patients in the comfort of their own homes, helping them work toward greater independence, functionality, and quality of life. This role offers the opportunity to build meaningful relationships with patients and families while providing personalized care and education. The Therapy Assistant works closely with the licensed professional and care team to support each patient's individual goals and Plan of Care.
What you will do as a Licensed Therapy Assistant:
- Conduct patient visits in a timely, professional, and appropriate manner
- Provide care and utilize therapy equipment according to the established Plan of Care and goals
- Monitor and document patients' responses to therapy goals and report changes in condition to the licensed professional and provider
- Provide education and guidance to patients and families on prescribed activities that promote independence and functionality
- Coordinate care with healthcare providers, office staff, physicians, and other members of the care team
- Collaborate with other healthcare professionals to promote continuity of care
- Help ensure patients and families have the resources they need to support their health and independence
- Participate in weekly case conferences, staff development meetings, and continuing education
What we are looking for in a Licensed Therapy Assistant:
- Graduate of an approved clinical education program
- Current license in the state where you practice
- At least 1 year of experience as a licensed professional
- Home Health or Hospice experience preferred
- Strong communication, decision-making, and critical thinking skills
- Comfortable using EMR systems and technology
- Current driver's license and reliable form of transportation
