Responsibilities:
GENERAL PURPOSE:Accountable for the provision of proficient patient care inclusive of complex patients. Assumes an expanded charge nurse role and assumes unit responsibility for orientation for new staff. May assume the role of care coordinator for a given group of patients, monitoring and evaluating patient care based on the unit standards of care. Actively involved in unit/service activities, contributing to outcomes from those groups.
FT position available, 40-hour position, 8/10 hour shifts primarily days.
Seeking a highly motivated and organized individual, to take on the role of patient experience coordinator for Interventional Radiology, which is a fast-paced and rapidly expanding procedural area. This individual will be responsible for performing pre-procedure screening and educational calls to patients prior to their procedure to ensure the patient arrives prepared. Assuming the role of care coordinator for a given group of patients, monitoring and evaluating patient care based on the unit standards. Accountability for the provision of proficient patient care inclusive of complex patients. This role must be flexible and work collaboratively with leadership and members of the multi-disciplinary team at SMH to assist in the delivery of an exceptional patient care experience. Tracking and reporting of service metrics will be required, in regard to patient cancellation, no show data, and patient satisfaction. ACLS is required within a year.
