STATUS: 聽聽Non-exempt (Full-Time), 37.5 hours, no after-hours or on-call duties are required

HOURLY RATE: up to $24.00 per hour

SUMMARY

This role provides enhanced care management to victims of elder abuse and mistreatment. The Upstate Elder Abuse Center (UpEAC) delivers crisis intervention and safety support through its Elder Abuse Prevention and Fraud and Scams Prevention programs, as well as Enhanced Multidisciplinary Teams (E-MDT) across 13 counties. The Elder Abuse Enhanced Intervention Services (EIS) program supports UpEAC clients needing longer-term case management.


The Care Manager connects older adults to available community resources, coordinates services, and provides guidance navigating complex systems while supporting client self-management.聽


DUTIES AND RESPONSIBILITIES

1.0 聽聽聽聽Provide care management for the Enhanced Intervention Services (EIS) project.

路 Accept referrals as assigned.

路 Maintains a caseload of clients referred to program services.

路 Provides ongoing information and referral, advocacy, benefit and entitlement counseling.

路 Completes comprehensive in-home assessments to determine needs.

路 Implements client-centered care plans that meet client needs and preferences; modifies service plans as required by changes in client circumstances.

路 Records case activity in the Lifespan database designated for the project.

路 Maintain contact with the clients at appropriate frequency to meet their needs and assess their condition or situation.聽


2.0 聽聽聽聽Arranges mental health interventions for older adults and their caregivers, when necessary.聽

路 Conducts mental health screenings when appropriate.

路 Provides supportive counseling to older adults.

路 Identifies and provides linkage to appropriate ongoing mental health community services.聽


3.0 聽聽Participates as a member of the inter-agency team.

路 Participate in all Upstate Elder Abuse Center team meetings and trainings.

路 Participates in all Lifespan staff meetings and trainings.

路 Participates in quality assurance activities.

路 Provides input into the continued development of this innovative program.

路 Maintains timely and accurate documentation.

路 Accepts and seeks guidance and clinical supervision.


4.0 聽聽Maintains up to date professional knowledge of the geriatric field, elder justice, community services, entitlement benefits, clinical skills and competencies through reading, workshops, and conferences.

路 Participation in training required for grant project activities.


5.0 聽Is aware of and actively supports the agency鈥檚 initiative of being collaborative and community focused, innovative and culturally competent.


6.0 聽Other duties as assigned by supervisor.

QUALIFICATIONS

EDUCATION: Bachelor or Master of Social Work degree.

EXPERIENCE: At least one year of experience coordinating community-based services for older adults.聽


PHYSICAL AND MENTAL REQUIREMENTS

路 Sound organizational skills

路 Must be able to meet with clients in their homes.

路 A portion of the work is performed at a desk or at a personal computer station.

路 Verbal, written and telephone communication skills are required.

路 Strong written and computer skills needed.

路 Interface with all levels of personnel.

路 Ability to work with older adult clients at risk of abuse, neglect, exploitation and self-neglect.

路 Ability to maintain composure under strict deadlines and difficult situations.

路 Ability to travel by car. Must have reliable transportation and valid driver's license to travel within the 13-county service region.

路 Ability to work in multi-cultural situations.

Salary/Hourly Rate

Multiple factors will be considered for salary/hourly determination including, years of relevant service, skills required, education, and internal equity.聽
