SUMMARY
Licensed Practical Nurse will provide home health services under the direction of a Registered Nurse to the Rochester Regional Certified Home Health Agency (CHHA) clients per Medicare, Medicaid and NYS Department of Health (DOH) regulations. Provide superior customer service by modeling the Brand Promise and Core Values.

RESPONSIBILITIES
Completes accurately and timely all documentation of assigned clients’ care per Federal and State regulatory standards.
Participates in the CHHA Service Education Program. Maintains updated knowledge base of community health practice and community resources.
Participates in the holiday/evening/weekend rotation.
Observes and documents signs and symptoms related to patient’s health status and changes in their condition and reports this to the registered nurse. Obtains the registered nurse’s assessment and signature on all assigned patients.
Contributes to the identification of basic physical, emotional and cultural needs of the patient through observations and interactions.
Provides routine nursing services, information and education to patients during routine care.
Collaborates with the appropriate registered nurse to revise care plans when the patient’s needs are not being met or have changed.
Maintains clinical competency.
Performs other duties as assigned.
REQUIRED LICENSURE / CERTIFICATION & SKILLSÂ Â Â
Graduate from an accredited School of Practical Nursing.   Â
Current LPN licensure in State of New York.   Â
Basic Life Support (BLS) Certification from the American Heart Association (AHA).  Â
A valid New York State Driver’s License and the ability to pass a driver’s license record background check.Â

Drive - Clinical - Roles in this category require frequent driving combined with mobile clinical care across settings such as emergency response, mobile clinics, and home health environments. Staff must repeatedly enter and exit vehicles, occasionally lift and carry equipment weighing up to 100 lbs. or more, and perform care within patients’ homes or other confined, unpredictable, or uncontrolled environments. These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery. Emotional and cognitive demands are substantial due to rapid decision‑making, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting. Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. Valid licensure, physical readiness, and flexibility for extended or on‑call schedules are typically required.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.
