Under the direction of the Manager, Quality, the Quality Specialist supports the Federally Qualified Health Center’s (FQHC) Quality Assurance and Performance Improvement (QAPI) program by analyzing quality data, monitoring clinical quality metrics, and partnering with interdisciplinary clinical teams to close care gaps.

This role translates data into actionable improvement strategies, supports patient outreach and engagement, and drives continuous quality improvement initiatives. The Quality Specialist plays a critical role in improving patient outcomes through sustainable workflow changes, with a strong focus on health equity, population health, and regulatory measures including HRSA Clinical Quality Measures (CQMs), Uniform Data System (UDS) reporting, and value-based care metrics.

Duties/Responsibilities:

  • Support clinic-level and organizational QAPI activities through data analysis, reporting, and interpretation of quality metrics
  • Collaborate with providers, nurses, care managers, and leadership to translate data into actionable improvement plans
  • Participate in clinic quality meetings to review trends, identify root causes, and recommend strategies
  • Serve as a liaison between Quality and clinical teams to support implementation of initiatives
  • Participate in Performance Improvement Projects (PIPs) using methodologies such as PDSA cycles
  • Analyze care gaps and performance variation across providers, sites, and populations
  • Document quality improvement activities for leadership and regulatory reporting
  • Partner with EHR/data teams to ensure accuracy and usability of reports and dashboards
  • Support workflow standardization to sustain improvements

Care Gap Outreach & Patient Engagement:

  • Conduct outreach to patients regarding preventive screenings and chronic care gaps
  • Assist patients with scheduling follow-up care
  • Escalate barriers to care to appropriate team members
  • Document outreach activities in the EMR
  • Maintain confidentiality and comply with organizational standards
  • Participate in audits, compliance activities, and continuous improvement efforts
  • Perform other duties as assigned

Required Skills/Abilities:

  • Understanding of health equity, social drivers of health, and population health principles
  • Ability to manage multiple priorities in a fast-paced environment
  • Excellent written, verbal, and computer skills

Education and Experience:

  • Minimum 2 years of experience working as a medical assistant in a clinical setting required
  • Federally Qualified Health Center (FQHC) and Family Medicine experience preferred.
  • Strong analytical and data interpretation skills with the ability to translate data into action.
  • Experience working with clinical quality metrics, dashboards, or EHR-based reports; strongly preferred.

Physical Requirements:
 While performing the duties of this job, the employee is regularly required to sit, stand, and walk within clinic and office environments. The role requires frequent use of a computer and phone, including typing and data analysis. The employee must be able to communicate effectively with patients and staff verbally and in writing. Occasional bending, stooping, and lifting up to 25 pounds may be required.
