Duties and Responsibilities:Ā Ā
- Facilitate calendar planning meetings.Ā
- Use a person-centered planning process, and individual needs and preferences are the driver of all activities. Ā Ā
- Assume leadership responsibilities for delivery of Day Habilitation services.Ā
- Facilitate the daily activities of the Day Habilitation program.Ā Provide support to staff and support personnel who assist in providing Day Habilitation services.Ā
- Arrange and provide transportation for My Day activities.Ā
- Facilitate community integration experiences to support fuller participation in community life.Ā Ā
- Perform 30, QA checks monthly on My Day documentation.Ā Ā
- Provide Support Team with input into individual service objectives, teaching procedures, service strategies and program ideas.Ā
- Complete daily documentation that is in compliance with federal and state regulations for Medicaid waiver programs for each client served in the program.Ā Ā
- As a resource to the Support Team, assist in managing funding of Day Habilitation services. Ensure accurate billing of services.Ā Act as a liaison to My Day Specialist and manager for schedule changes that would affect funding.Ā Ā
- Ensure Performance Measurement System data for the Day Habilitation program is accurate and up to date. Provide input for the annual performance measurements report for Day Habilitation program.Ā
- Provide client assistance with general duties such as promoting and assisting with grooming and hygiene, restroom usage, hand/face washing, mealtime assistance and supervision, transportation supports and escorting assistance as needed, etc.Ā Ā Ā
- Follow all safety procedures and promote safety measures among the team and clients.Ā Implement emergency drills and complete tracking sheets.Ā

Qualifications:
- TwoĀ years' experienceĀ in working with people with developmental disabilities and BA in Special Education or Human Services field preferred.Ā
- Skill in Behavior Management.Ā
- Skills in computer use and word processing.Ā
- Knowledge of and ability to utilize assistive technology and adaptive equipment.Ā
- Valid Class C driverās license required, unless driving a vehicle with a capacity of 8 or more, then a Class āDā license will be required and be an insurable driver with our insurance company.Ā
- Reliable personal vehicle and proof of state minimum insurance coverage.Ā Ā Ā
- Up to date physical.Ā
- Clearance from all applicable background checks as required by service standards.Ā
