OVERVIEW
The Case Manager/Care Coordinator helps older adults and their families access community resources and long-term care services by conducting in-home assessments, developing individualized care plans, coordinating services, and determining program eligibility. This role serves as an advocate for participants, monitors changing needs, assists with Medicaid and other benefit programs, and helps ensure clients receive the support needed to remain safe and independent.

ESSENTIAL DUTIES AND RESPONSIBILITIES
- Performs comprehensive assessments in the participant’s home or health care facility to assess need for community services. Determines eligibility for CCP, accurately administers and scores Mini Mental Status Exam (MMSE) and Determination of Need (DON) and collects needed financial information.
- Creates a care plan to meet the needs of the participant and caregiver. Demonstrates ability to authorize CCP services and coordinate referrals to other benefits/services available to the participant.
- Completes Choices for Care assessments for Nursing Home or Supportive Living Facility placement
- Demonstrates knowledge of the various types of assessments, billing codes and documentation required.
- Accurately completes Medicaid applications when indicated.
- Advocates, intervenes and follows-up in a timely manner to ensure services remain in place and are appropriate to the changing needs of the participant. Documents and completes follow-up on critical events. Updates care plan and completes redeterminations as needed.
- Meets established timelines for assessments and critical event follow-up.
- Rotates weekend on-call duties as required by IDOA for weekend assessments.
- Completes paperwork timely and accurately. Responds to participant/family inquires and phone calls in a timely manner.
- Participates in departmental and all-agency staff meetings; offers constructive input to the group.
- Completes annual educational requirements for IDOA and Mercyhealth.
EDUCATION AND/OR EXPERIENCE
Case Manager minimum qualifications shall be a minimum of 1 year experience in needs assessment and care planning, and:
Bachelor's degree in social science, social work, nursing, or related field
One year of program experience, may replace one year of college education up to and including four years of experience replacing a baccalaureate degree; or
LPN with one year of program experience
CERTIFICATION/LICENSURE
BLS certification required within 90 days
Successfully complete IL Department on Aging Care Coordinator Certification
Maintain a valid driver’s license and reliable transportation with proof of vehicle insurance when requested.
Have and maintain an acceptable driving record per Mercy’s HR-01 Fleet Safety policy.
OTHER SKILLS AND ABILITIES
Basic PC/Laptop skills
Knowledge of Microsoft Office, Internet Explorer, Google Chrome
Excellent customer service skills
Effective verbal & written communication
Organized, able to multi-task in a fast-paced environment
Able to sit, stand and walk intermittently
Ability to lift/carry laptop and paperwork to/from office and homes/facilities
Must be able to navigate various entrances to homes/apartments/facilities
