We are currently seeking an experienced:

HOSPICE LIAISON

Full Time

(Covering Franklin, Nash, Wilson, Edgecombe Counties)

Job Summary:

  • Perform hospice education activities at health care institution and physician office
  • Coordinate patient services.
  • Coordinate hospice education activities in the community
  • Coordinate with health care institution and physician education activities.
  • Interact with Administrator.

Job Requirements:

  • Requires a Bachelor’s degree, RN, or MSW.
  • Prefer two years’ experience working with home health/hospice, hospitals, physicians, HMO’s, outpatient services or social services concerning public relations or office procedures. Knowledge of Medicare/Medicaid hospice regulations preferred.
  • If licensed, successful completion from an approved school of nursing or college education.
  • Ability to coordinate the transfer of patients to hospice services, resulting in uninterrupted continued medical care.
  • Must possess a strong knowledge of hospice services, comprehension of federal, state and ACHC guidelines.
  • Assist with continuous recruitment activities to attract qualified staff.
  • Effective communication skills (oral and written).
  • Strong interpersonal and telephone skills.
  • Ability to travel as necessary.
