Rome Health is seeking a Discharge Planning Referral Specialist. This position will support the entire Continuum of Care team, including the directors, Case Managers, Social Workers, and other members of the health care team in meeting discharge planning and continuity of care needs for patients and their caregivers. This position will support completing the concurrent insurance reviews on the Rome Behavioral Health (RBHU) unit and the management of referrals to the skilled nursing facilities and home care agencies via the Care Portal. Duties will also include:

  • Playing a pivotal role in managing patient referrals.
  • Ensuring effective communication with referral sources and supporting care managers throughout the intake process.

EDUCATION, TRAINING, EXPERIENCE, CERTIFICATION, AND LICENSURE:

Associate degree or higher in a health-related field


KNOWLEDGE AND SKILLS REQUIRED:

Basic knowledge of Case Management and Social Worker principles and practices. Knowledge of office terminology, procedures and equipment. Entry level of knowledge of medical terminology, business arithmetic, and English.

Excellent written/oral communication, interpersonal, motivational and strong organizational skills, including time management skills required. Must have the ability to prioritize and follow oral and written instructions. PC and appropriate software knowledge.
