Job Description

Meets all requirements of a RN Level II.

Accurately assesses all wound, ostomy, and continence patients. Documents all pertinent wound/ostomy/continence information in the medical record. This includes, but is not limited to, wound measurements, descriptions, photographs, current treatment regimen, and interventions in place.


Accurately inputs charges for procedures in the medical record.


Uses critical thinking skills and evidence-based practice in the evaluation and selection of patient care products.


Demonstrates proficiency with the application of wound dressings.


Demonstrates proficiency with the evaluation of ostomies and ostomy appliances.


Professionally communicates with and educates the patient care team.


Professionally communicates with and educates the patient and their family or other caregivers.


Collaborates with the patient care team and establishes a plan of care using evidenced based practice. This also includes discharge planning, education, and DME supplies.


Serves as a consultant and nurse expert providing evidence-based guidance and instruction on complex wound, ostomy and continence issues to peers, hospital committees, other entities, and home health agencies.


Seeks out or maintains certification. Actively participates in continuing education and stays abreast of best practices in the field.

Education
Associates Nursing (RN Diploma, ASN or ADN) required.


Bachelors Nursing (BSN) preferred.

Experience

Minimum of 5 years of clinical RN experience required.

Certification & Licensures
Current licensure or licensure eligible as an RN in the state of WV.


Wound, Ostomy, and/or Continence Nursing Certification required within 24 months of hire.


BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required. New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.

Qualifications
Computer literacy required.
