Adapt Integrated Health Care is looking for a Licensed Practical Nurse (LPN) to join our Residential team! 

The Licensed Practical Nurse will assess patient health problems and needs throughout the duration of their treatment. Develop and implement nursing care plans and maintain medical records.  Administer nursing care to patients and advise on health maintenance and disease prevention or provide case management. The position will supervise medical staff and assist in treatment by also monitoring vital signs, dispense medication at the correct times, report to the medical staff concerns, and provide top notch customer service.

Qualifications:

  • Valid LPN in Oregon with no restrictions
  • 2 years’ experience in acute care, critical care, emergency room or correctional health care setting preferred.  
  • CPR, First Aide, AED, Universal Precaution experience a must.

Primary Responsibilities:

  • Oversee admission to the program i.e., medically clear clients for scheduling, communicating closely with Healthcare Services Coordinator and processing client at intake. 
  • Demonstrate therapeutic, trauma informed care approach with clients.
  • Administer urinalysis, urine hCG, BrAC, and TB and document results, communicating results to provider.
  • Assess, monitor, and treat clients according to provider orders and department protocols.
  • Document assessments, interventions, and acute changes in client behavior and condition. 
  • Maintain accurate, timely, and detailed reports and records.
  • Communicate client information to rest of the team and provider as appropriate to facilitate coordination of care.  
  • Educate clients. 
  • Direct and supervise LPNs and CCMAs, considering scope of practice. 
  • As necessary, other duties to support client care and unit functioning.
  • Attend meetings and staffing as needed/required.
  • Maintain confidentiality of all client information.
