  • 100% Outpatient - primary focus on Geriatrics patient-populatio
  • Preventative care focus on Chronic conditions to reduce hospitalization
  • Well trusted, Physician-lead healthcare org. with over 120+ locations in 15 states
  • Patient-focused, small patient panel – 450 patients max
  • Monday – Friday shift-schedule; No Weekend, No Call
  • Dedicated care team for full PCP support, as well as Onsite lab, Pharmacy, and X-Ray
