As a Disability Support Professional, you’ll be a key part of our team, helping empower residents to live fulfilling lives!


This is a full-time position with a rich benefits package.

As a valued care team member, you’ll play a vital role in creating a supportive and enriching environment for our residents.

Here’s how you’ll contribute:

  • Be a Care Partner: Provide compassionate assistance and guidance with daily living activities, such as dressing, grooming, and mealtimes, fostering a sense of dignity and self-reliance.
  • Facilitate Engaging Experiences: Engage our residents in meaningful activities according to their individual programs and group experiences to promote their well-being.
  • Help Build Skills: Guide residents in building new skills, maintaining existing abilities, and actively participating in community inclusion activities.
  • Be a Positive Role Model: Use positive reinforcement to motivate and encourage residents and fellow staff members.
  • Be a Safety Advocate: Proactively address any concerns and adhere to Horizons’ policies and procedures to ensure a safe and healthy environment.
  • Be a Team Player: Foster a collaborative and supportive atmosphere by communicating effectively with colleagues and contributing to conflict resolution.
  • Be a Problem Solver: Confidently handle unexpected situations, making sound decisions in emergencies and escalating concerns to supervisors when needed.
  • Be a Documentation Specialist: Accurate and timely documentation is vital to ensuring the continuity of care for our residents. Your detailed records will directly contribute to their well-being and progress.

What you’ll bring to the Team:

  • A genuine passion for working with individuals with disabilities.
  • A commitment to providing person-centered care with respect and dignity.
  • The ability to lift, push, pull, and move 40 pounds to assist with resident mobility.
  • Great communication and interpersonal skills.
  • A proactive and solutions-oriented approach.
  • A willingness to participate in ongoing training and professional development.

Qualifications & Skills

Required:

  • Must be at least 18 years of age and have a high school diploma or equivalent.
  • Must possess excellent written and oral communication skills

Preferred:

  • Prefer at least one year of pediatric or long-term care experience with knowledge of ICF (intermediate care facility)
  • Preferred certification in Adult and Pediatric CPR, First-Aid, AED, and BLS
  • Proficient in electronic medical documentation
  • Valid NC driver’s license

Job Type: Full-time, Second Shift 2:45pm - 11:15pm
