At SCH, we appreciate that having employees of many different backgrounds strengthens our organization, and we strive to create a workforce that reflects the diverse and culturally rich communities that we serve.   OVERVIEW:  Under the guidance of the Clinical Supervisor, the Family Specialist functions as a member of an assigned service team within the FSPW program and provides direct services to children and families as identified in individualized service plans.   DUTIES:   1. (75%) Direct Services Engage with assigned families, in accordance with individualized service plans or as determined by the Child and Family Team (CFT) meeting, in providing concrete services to assist the child and family with their tasks of daily living. Provide services and interventions to include but are not limited to addressing needs in the areas of education, parenting , housing and home management, recreation, and behavior/emotions. As directed by facilitator, develop individualized rehabilitation plans in alignment with service goals and objectives outlined in service plans. Implement High-Fidelity Wraparound and other evidence-based interventions to address physical, cognitive, emotional, and social rehabilitation needs. Monitor progress and adjust rehabilitation plans as necessary. Support and education to families regarding rehabilitation strategies and resources. Provide advocacy for and support and intervention to individual family members and the family. Assist in providing crisis management to families and children when plans disrupt and on-site consultation to others involved in plans. Collaborate with team to assess the rehabilitation needs of children and families. Foster a supportive and empowering environment for children and families to achieve established goals. Coordinate with external providers, such as school staff, probation officers, child welfare workers, Alta Regional providers, etc., to ensure comprehensive care for the child’s needs. Utilize existing strengths and strengths to build to advocate for accessibility and inclusion of the child in community activities and resources. Assist in the ongoing development and evaluation of the service program. Participate in the Wraparound on-call system as needed.   2. (25%) Other Attend and participate in all team meetings, child and family meetings, and other meetings as required. Participate in interdisciplinary team meetings (i.e., MDT, TA) to discuss progress, challenges, and adjustments to rehabilitation plans. Establish and maintain collaborative and effective working relationships with internal staff throughout all levels of the agency and with external resources. Collaborate with other team members to ensure seamless integration of rehabilitation services with other direct services provided. Foster a supportive and empowering environment for children and families to participate in their own journey actively. Participate in training activities as directed. Assist in monitoring and ensuring effective linkages with community and agency service providers. Assist in completing all documents for the Wrap program, including but not limited to tracking sheets, family safety plan, family review, family budget, assessment plans, etc. Maintain accurate and timely documentation in accordance to agency policies. Serve as a role model and advocate for children and families involved in the Wraparound process. Stay informed about current trends, best practices, and advancements in rehabilitation services through ongoing training and professional development. Adhere to ethical and professional standards in all aspects of service delivery. Perform other duties as assigned.   Knowledge, Skills, and Abilities: Ability to read, write, speak, and understand English. Ability to establish and maintain appropriate boundaries with families. Ability to work flexibly, creatively, and comfortably with families of varied ethnic and socio-economic backgrounds. Knowledge of child abuse issues. Knowledge of child development and behavior. Knowledge of family dynamics. Knowledge of substance abuse issues. Knowledge of local cultures. Skill to communicate effectively in written and oral form. Ability to manage multiple tasks at one time in an efficient manner. Good organizational skills. Skill to maintain a professional, confidential work environment. Excellent problem-solving skills. Knowledge of Microsoft Word. Ability to report to work on a regular and reliable basis.   LOCATION:   2750 Sutterville Rd., Sacramento, CA 95820   SCHEDULE:   Monday – Friday, 9:00am – 5:30pm, evenings and/or weekends as needed                            SALARY:         $22.85 – $25.70 per hour, to start QualificationsDriving is an essential function of this job, and as such, employees in this position will be required to furnish their own vehicle to be used in the course of employment. A valid driver’s license and minimum insurance coverage is required.   Must be at least 21 years of age.   Education and Experience:   At least one year of full-time experience working in the mental health field is required.
