Physical Therapist

We’re seeking a compassionate and motivated Physical Therapist to join our growing team of clinicians dedicated to delivering exceptional, patient-centered care in the comfort of patients’ homes. What You’ll Do

• Conduct initial patient visits, including comprehensive assessments and evaluations
• Develop and implement individualized treatment plans
• Supervise and collaborate with Physical Therapist Assistants (PTAs)
• Coordinate care with interdisciplinary team members
• Monitor patient progress and adjust care plans as needed
• Complete timely and accurate documentation
• Lead patient discharge planning and education

What You Bring

• Current, active Physical Therapist license in the state of employment
• Valid driver’s license, auto insurance, and reliable transportation
• Current BCLS/CPR certification
• Home health experience preferred, but not required
