Regular operating hours Monday-Friday 8:00 am-8:00 pm, Saturday/Sunday 9:00 am-5:00 pm. This position requires an every third weekend rotation.

We are seeking a dedicated and compassionate Consultant Pharmacist to join our team and provide essential support to our long-term care pharmacy. This position is responsible for delivering exceptional customer service while conducting comprehensive clinical reviews that support quality resident care. The role requires frequent independent judgment, strong critical‑thinking skills, and thorough knowledge of state‑specific regulations, federal CMS guidelines, and organizational compliance standards. In addition, this position plays a vital role in ensuring safe, effective, and compliant medication management for older adults living in Ebenezer communities. The work blends clinical expertise with regulatory insight and close collaboration across interdisciplinary teams to enhance resident outcomes, strengthen care coordination, and support smooth, person‑centered care transitions.
Responsibilities
- Perform daily clinical evaluations including admission, interim, comprehensive, and ongoing medication regimen reviews, as well as targeted disease‑state management assessments to ensure safe, effective, and compliant medication practices for residents.
- Conduct monthly or as needed resident medication reviews based on acuity, providing written reports to facility leadership ( administrator , director of nursing, and prescribers).
- Maintain a working knowledge of state and federal pharmacy regulations relevant to long term care practices.
- Serve as a liaison between facility and pharmacy operations teams to promote communication and service excellence.
- Demonstrate understanding of CMS regulations and interpretive guidelines.
- Participate in quality assurance and interdisciplinary committee meetings as requested.
Required Qualifications
- Doctorate of Pharmacy
- Comprehensive knowledge of pharmacy practices and procedures.
- Ability to use Microsoft Office Suite, including Excel and Word.
- Mathematical abilities such as calculating discounts, interest, commissions, proportions, percentages, area, and volume; basic algebra; practical mathematics.
- Pharmacist MN Board of Pharm
- Basic Life Support (American Heart Assoc or Red Cross)
- Immunization Certified in State of MN within 30 Days
- Drivers License in good standing
Preferred Qualifications
- Previous consultant pharmacist experience in LTC setting
