SUMMARY:

The Licensed Hospice Social Worker works as a member of the interdisciplinary team providing social services to terminally ill patients and their families, in accordance with the each patient’s plan of care, and under the direction of a physician. The Social Worker functions as one of the four core Hospice team members of the Hospice Inter-disciplinary Group (IDG).

JOB SPECIFICATIONS:

Education, Experience and Certifications

  • Bachelor’s degree in Social Work or related field required.
  • Must possess Licensed Master of Social Work (LMSM).
  • One or more years of clinical social work experience with individuals and families required, hospice experience preferred.
  • CPR Certification required.

Compensation: An individual's pay rate within the posted range may be determined by various factors, including skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization prioritizes pay equity and considers internal team equity when making any offer. Hiring at the maximum of the range is not typical. Shift and/or other differentials may be available to increase your pay rate for certain shifts or work.

Internal POOL
-Provide support for hospice and home health patients in 7 county metro service area. Internal pool position to support team in absence of primary Social Workers and in times of increased work load.
Covering 8am - 5pm (POOL - Hours may vary)
